Comprehensive Functional Model of Care Improves Outcomes in Older Adults

Functional decline during hospitalization is an important clinical problem with potential long-lasting adverse outcomes in older adults.

Approximately 50% of hospitalized older adults experience functional decline.

The implementation of a comprehensive functional model of care for hospitalized older adults has had a positive impact on length of stay (LOS), 30-day readmission rate, and fall rate.

Estimated savings associated with the 3-month post-intervention LOS was $200,000.
